Attention Weaver Elementary CHAIRPERSONS & VENDORS

Below is a link to the California PTA required insurance forms if you will be providing ANY service for ANY PTA related event whether it is on campus or at another location.

All Vendors must submit this paperwork to their Main contact along with a signed HOLD HARMLESS agreement,

prior to the scheduled event. If you are a NEW vendor and have not provided your w-9, you must send your W-9 to your main contact or to weavertreasurer@yahoo.com

 

 

 


Weaver PTA Chairs & Committees: 

Below is a link to the PTA reimbursement form. 

Use this to be reimbursed for expenses for all PTA related events.

 

Your budget/plan must have been presented and approved by the board and general membership prior to being reimbursed for any expenses. If you are unsure if something has been approved or a budget line item release please contact weavertreasurer@yahoo.com or trishweaverpta@gmail.com

Once completed, email the form and all receipts (scanned) to,

weavertreasurer@yahoo.com

 or please drop off the physical copy of the form and the physical receipts in the REIMBURSEMENT basket in the school office.
